Our intake and portal run on AdvancedMD, which is built and secured for exactly that under HIPAA. You will see the address change when you get there. That is expected, and it is a good sign. Our logo is at the top of the form, and everything you send arrives with our team.
Begin your care →About 15 minutes, in one sitting. Have your insurance card, your medication list and your pharmacy name with you. Use your phone if you can, because you will photograph the card.
